1. Dr Miller's team - Dr Miller is typically in clinic on Tuesday and Thursday.
Dr Jolley is typically in clinic on Friday. This may change from time to time.
2. Nurse Practitioner Emily V for all Hope Health Patients
Online booking is not currently available for Dr Zajdlik's team including Dr Khan or Nurse Practitioner Christine Sanford
When booking online, please note:
- same day appointments can not be booked online, please call the clinic at 519 822-6700 for urgent concerns
- most other appointments are available online - calling the clinic will rarely result in a faster non-urgent appointment
- new issues must be booked as in person assessments - if you have a new issue that you feel can be handled by phone or video, please call the clinic to discuss
- follow-up issues requiring a physical exam (eg rash, blood pressure check, diabetes check, back pain) should not be booked as a phone or video appointment unless you have been told in advance that it is okay
- nursing appointments can not be booked online
- any comments, notes or information you provide when booking will not be read until the day of your appointment, please do not use this feature to make requests in advance of an appointment
